Ganzoni Equation (IV Iron Dose)

Calculate total IV iron deficit (mg) using the Ganzoni formula. Critical for Indian practice — over 50% of women and 70% of children have iron deficiency anemia.

Suggested Regimens (Indian brands)

Iron Sucrose (Orofer-S, Encicarb): 7 sessions × 200 mg IV slow over 30 min, every 2-3 days. Cheapest option (~₹250-400/vial).

Ferric Carboxymaltose (Ferinject, Encicarb-FCM): 2 dose(s) × up to 1000 mg single infusion over 15-30 min. Premium option (~₹3,500-5,000/vial). Watch for hypophosphatemia.

Iron Isomaltoside (Monofer): 1 dose(s) — full deficit in single infusion up to 1500 mg over 30 min. Good safety profile.

What is the Ganzoni equation?

The Ganzoni equation (1970) calculates the total body iron deficit (in mg) required to restore hemoglobin and replenish iron stores. It is the standard formula used worldwide to dose parenteral iron in iron deficiency anemia (IDA).

Formula: Iron deficit (mg) = body weight (kg) × (target Hb − actual Hb) (g/dL) × 2.4 + iron stores (500 mg if >35 kg; 15 mg/kg if ≤35 kg)

Why this matters in India

  • India has one of the highest IDA prevalence rates globally — NFHS-5 shows 57% of women 15-49y, 67% of children 6-59m, and over 50% of pregnant women are anemic.
  • Oral iron is first-line but causes GI intolerance in 30-40% — many patients abandon therapy within 2 weeks.
  • IV iron achieves rapid Hb correction (1-2 g/dL rise in 2 weeks) — invaluable in late pregnancy, pre-surgery, post-partum hemorrhage, and CKD.
  • FOGSI 2022 recommends IV iron for Hb <9 g/dL in pregnancy at >20 weeks gestation.

Choosing between Iron Sucrose, FCM and Isomaltoside

  • Iron sucrose (Orofer-S, Encicarb): Cheapest. 200 mg per session, max 3×/week. Best for low deficits (<1000 mg) or budget-constrained patients.
  • Ferric carboxymaltose (Ferinject): Up to 1000 mg single infusion. Convenient (1-2 visits). Watch for hypophosphatemia with repeated dosing.
  • Iron isomaltoside (Monofer): Up to 1500-2000 mg single infusion. Lowest risk of hypersensitivity. Premium pricing.
  • Avoid IV iron in first trimester of pregnancy unless severe IDA & oral intolerance.

Frequently Asked Questions

When should I use IV iron instead of oral iron?

When Hb is below 9 g/dL with symptoms, when oral iron is poorly tolerated or ineffective after 4 weeks, before elective surgery (Hb <11 g/dL), in late pregnancy needing rapid correction, in CKD patients, post-bariatric surgery, and in chronic blood loss conditions.

Can I give the full Ganzoni deficit in one sitting?

Yes with FCM (up to 1000 mg) and Iron Isomaltoside (up to 1500-2000 mg). Iron sucrose is limited to 200 mg per session because of higher labile iron release.

What is the expected Hb response after IV iron?

Hb rises by 1-2 g/dL in 2-3 weeks. Recheck CBC at 4-6 weeks. If response is poor, look for ongoing blood loss, infection (anemia of chronic disease), or wrong diagnosis (B12 / folate deficiency, thalassemia).

Is IV iron safe in pregnancy?

Yes, after the first trimester. FOGSI 2022 and WHO endorse IV iron in 2nd/3rd trimester for moderate-severe IDA. Avoid first trimester unless severe disease.

How much does IV iron cost in India?

Iron sucrose: ₹250-400/vial (200 mg). Ferinject: ₹3,500-5,000/vial (500-1000 mg). Monofer: ₹4,000-7,000/vial. Day-care administration charges extra.

Clinical Disclaimer: Always rule out blood loss, malabsorption (celiac, H. pylori) and chronic disease before attributing anemia to iron deficiency alone. IV iron requires monitored infusion — test dose for iron sucrose, watch for hypersensitivity with FCM. Always verify against your local prescribing reference and apply clinical judgment.
