What is PEWS?

The Pediatric Early Warning Score (Brighton PEWS) is a bedside scoring system that combines behaviour, cardiovascular and respiratory parameters into a single score (0–9) to identify children who are at risk of clinical deterioration. Higher scores correlate with PICU admission, cardiac arrest and mortality.

PEWS is now embedded in NHS, AAP and IAP-endorsed pediatric early warning systems and is required by NABH/JCI accreditation for pediatric wards.

When to calculate PEWS

  • At every nursing observation set (typically 4-hourly on wards).
  • At ED triage for any child looking unwell.
  • Post-operatively in the first 24 hours.
  • Whenever parents say "my child looks different" — parental concern can be the strongest predictor.

Age-specific normal vital signs (IAP)

  • < 1 year: HR 110-160, RR 30-60
  • 1-3 years: HR 90-150, RR 24-40
  • 4-5 years: HR 80-140, RR 22-34
  • 6-12 years: HR 70-120, RR 18-30
  • > 12 years: HR 60-100, RR 12-20

Indian context — implementing PEWS in tier-2 hospitals

India's pediatric hospital mortality remains high (~3-5% in tertiary centres) partly due to delayed recognition. PEWS implementation in Indian centres (AIIMS, CMC Vellore, PGIMER) has reduced unplanned PICU admissions by 30-40%. Practical points:

  • Print laminated age-band cards (HR/RR) at the bedside.
  • Train staff nurses on the "parent concern" trigger — culturally Indian parents often hesitate to escalate.
  • Use PEWS as a hand-off tool during shift changes.
  • Audit PEWS-triggered escalations monthly for the M&M committee.

Frequently Asked Questions

How is PEWS different from qSOFA or NEWS2?

PEWS is pediatric-specific and uses age-band normal ranges. qSOFA and NEWS2 use fixed adult ranges and would massively over-trigger on a child's normal tachycardia. Always use a pediatric tool for children < 16 years.

Should I use PEWS or PEWS2 or Pediatric NEWS?

Brighton PEWS (in this calculator) is the most widely validated and adopted. PEWS2 (UK 2023 update) adds parent concern and oxygen saturation as separate triggers — equally valid. Pick one tool, train your unit, and audit consistency — that matters more than the specific version.

Does a PEWS of 0-2 rule out sepsis?

No. PEWS is a screen, not a rule-out. Early septic shock can present with normal vitals (compensated stage). Always combine PEWS with clinical assessment — capillary refill, mottling, parental concern, lactate.

Is PEWS recognised by NABH for pediatric ward accreditation?

Yes. NABH 5th edition and JCI standards for pediatric care require a documented early warning system with escalation pathway. PEWS satisfies this requirement. Audit logs are reviewed at NABH surveillance visits.

What about neonates?

Brighton PEWS is for children > 1 month. Neonates have their own scoring systems (NEWS, SCAMPS) due to very different vital sign ranges. Use a dedicated neonatal score in your NICU.
