Indications

Adult Dosing

Acne vulgaris

0.025% cream or gel — apply a thin pea-sized amount to affected area at night

Once nightly; long-term use (at least 12 weeks for full benefit)

Start with lowest concentration (0.025%) and increase to 0.05–0.1% as tolerated. Wash face, pat dry, wait 20–30 minutes, then apply (reduces irritation).

Photoaging / fine lines / hyperpigmentation

0.025–0.05% cream nightly to face

Once nightly; long-term — benefits seen at 3–6 months

Use moisturiser before/after to manage irritation. SPF mandatory in the morning. Combination with hydroquinone (Kligman formula) enhances effect for melasma.

Pregnancy & Lactation

Pregnancy: Category C

Topical tretinoin — systemic absorption is minimal but measurable. Oral retinoids (isotretinoin) are teratogenic; topical form carries theoretical risk especially in the first trimester. Generally advised to avoid, particularly in first trimester; use only if clearly needed with informed consent.

Lactation: Caution

Topical absorption is low. However, avoid application to breast area. Caution in breastfeeding — limited data on infant exposure.
