Indications

Adult Dosing

Depression

25–75mg at night

Start 25mg at night; increase by 25mg every 1–2 weeks to 75–150mg/night

Give as single bedtime dose to use sedation therapeutically and reduce daytime side effects. Maximum 150–300mg/day in hospital settings.

Neuropathic pain

10–25mg at night

Start 10mg; increase slowly to 25–75mg. Often effective at sub-antidepressant doses.

First-line recommended (NICE, NeuPSIG guidelines) for neuropathic pain

Migraine prophylaxis

10–25mg at night

Increase slowly to 75mg/night as tolerated; reassess after 2–3 months

Fibromyalgia

10–25mg at night

10mg starting; titrate to 25–50mg

Insomnia (off-label)

10–25mg

1–2 hours before bed

Maximum daily dose: 150mg/day (outpatient); 300mg/day (inpatient under specialist supervision)

Pediatric Dosing

Age Range: Nocturnal enuresis: 6–12 years
Dose: Enuresis: 10–20mg at night (6–10 years); 25–50mg at night (11–16 years)
Max/day: 50mg/night (children <11 years); 75mg/night (adolescents)

NOT recommended for paediatric depression (increased suicidality). Use for nocturnal enuresis only under specialist guidance. Duration limited; reassess need periodically.

Hepatic Adjustment

Use with caution in hepatic impairment; reduce dose. May precipitate hepatic encephalopathy. Avoid in severe hepatic disease.

Pregnancy & Lactation

Pregnancy: Category C

Neonatal withdrawal syndrome and respiratory depression reported. Anticholinergic effects may affect fetal heart rate. Use only if clearly indicated. Avoid near delivery if possible.

Lactation: Caution

Excreted in breast milk; relatively low neonatal exposure. Sedation in nursing infants reported. Monitor infant. Generally considered compatible with breastfeeding at low doses.

  • Paralytic ileus and urinary retention (from anticholinergic effects)
  • Manic switch in bipolar disorder
